Abundant Living Home Health LLC (“we,” “us”) respects your privacy. This policy explains what information we collect through abundantlivinghh.com, how we use it, and the choices you have. It covers our website and our work text messages. Health information we receive as a care provider is also protected under our Notice of Privacy Practices, which we give to clients when services begin.

Information we collect

• Free consultation form: name, date of birth, phone number, whether you have Medicaid or Medicare and which plan, and anything you choose to tell us in the message box.
• Job applications: contact information, date of birth, Social Security number, education, work history, references, availability, answers to background questions, consents and electronic signatures.
• Automatically: basic usage data such as pages visited, browser type and approximate location, collected through Google Analytics and cookies.

How we use it

• To respond to your request and check your eligibility for services
• To review job applications, verify references and complete criminal history and registry checks required by Texas law
• To operate, secure and improve our website

We do not sell your personal information.

Who we share it with

We share information only as needed to provide services or process applications, including: service providers that host our website and deliver form emails (such as our web host, WordPress form tools, and the secure U.S. cloud services that run our records system and send our work texts), Google (analytics and maps), background-check and registry sources required for hiring, Medicaid health plans when you ask us to request services, and government agencies when required by law. None of this sharing includes text message opt-in data and consent, or the mobile numbers we collect for our text messages, except with the providers that send those texts for us; see “Text messages” below.

Text messages

If you work for us or apply for a job with us, and you agree to get our work text messages, we collect your mobile number, a record of your agreement (the date, how you agreed and the wording you agreed to) and the texts we send you and you send us. We use them only to send the work texts you agreed to and to answer your replies. Texts never include client or health information. Message frequency varies. Message and data rates may apply. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text message opt-in data and consent will not be shared with any third parties, except the service providers that deliver our text messages for us, under contract and only to provide that service.

How we protect it

Our website uses encryption (HTTPS). Access to submitted forms and applications is limited to staff who need it. No method of transmission over the internet is completely secure, so please call us if you would rather share sensitive information by phone or in person.

How long we keep it

We keep information as long as needed for the purpose it was collected, and as long as required by state and federal record-keeping rules.

Your choices

You can ask us to review, correct or delete information you submitted through this website, subject to legal record-keeping requirements. You can block or delete cookies in your browser settings.

Children

Our website is not directed to children under 13, and we do not knowingly collect their information online.
